Forms and Information for Registered Campers

Camper information packets and our 2010 Health History/Emergency Treatment Form and 2010 Parent/Guardian Informed Consent Form will be posted when they are available. Check this page for these forms once registration began on Thursday, January 21, 2010.

The following forms (for campers who are already registered) must be sent to the camp office by April 1 or ASAP if registering after April 1:

• Parent/Guardian Informed Consent Form (if you register online, this step is completed electronically and you will not need to mail this form)
